$39,227 ($20.95/hr.) - $49,035 ($26.19/hr.) 

Summary of Job Duties
Assist residents in accessing services available in the community to promote independent living. Conduct comprehensive, non-clinical assessments of residents for wellness and social needs; assist residents in identifying, accessing, and coordinating services (such as personal care services), monitor services provided, and provide communication, as needed. Encourage and motivate residents to engage with providers and participate in their own care/services management. Develop and arrange educational/preventive health programs and services for residents. Develop and sustain partnerships with community-based supportive service providers and maintain an up-to-date resource directory of local service providers. Create and maintain resident files and program files. Submit Semi-Annual Performance Report, as required. Attend required training.

Minimum Qualifications
Bachelor's degree in social work plus 2 years of social work experience, preferably working with aged and disabled clients, or an equivalent combination of education and experience. Skill in using Microsoft Office Suite preferred. NOTE: New hires must have 36 hours of classroom/seminar training before or within 12 months of hire on age-related and disability issues, and 12 hours of continuing education each year.

• Valid Driver's License
• Valid Alabama Social Work license preferred
• Ability to be insured under the Authority's vehicle policy
